Dear Parents:

As we head into the summer season, I hope that you are beginning to relax and enjoy your family away from the sometimes overwhelming structure of the academic year. Before you do settle into summer, I would ask that you please read this letter and keep it for reference, as it does contain information that is critical to the start of the new school year.

Included in this mailing you will find an Annual Health Update Form and an Emergency Information Form. Please carefully check the printed information on the emergency form and make any necessary changes. (Be sure to include a telephone number where you can be reached in the event of an emergency and two other non-parent contacts.) If you would like to be contacted by e-mail, please provide us with your email address as well. (Since Emergency Information is vitally important, as is proof of physical examination, we must ask that you provide the school this information no later than July 15^th)   No student will participate in any sport or activity until all required forms have been returned to the school. Thank you very much for your help here.

As we did this past year, instead of mailing you a cumbersome packet of additional information, we have made the information accessible to you on the Cheverus web site. Although all of the information made available to you is important, I have listed below documents that require your immediate attention:

• SCHOOL POLICY requires that all freshmen, juniors, and transfer students provide documentation of a physical exam. Cheverus encourages using the athletic Pre-Participation Physical Form available on our website. The documentation will be filed in the student’s Permanent School Health Record.

• Athletic Pre-Participation Physical Form  The Athletic Department requires that any student who plans to play a sport during the 09-10 school year, have on file a current completed physical form(physicals are good for two years). Download the form. The fall sports pre-season begins on Aug. 17^th. 
• Substance Abuse Contract (Must be signed by all freshmen and transfer students) NO STUDENTS WILL PARTICPATE IN ANY ACTIVITIES OR RECEIVE THEIR FINAL COURSE SCHEDULE BEFORE SIGNING AND RETURNIG THIS DOCUMENT. (Due July 15^th) [Download the form.]
  
• Parent Directory Form   Sign and return this document if you do not wish to be included in the Student Directory.

Please review the “Parent Information” carefully so that you will be well prepared to start the new year. 

NOTE:  Download the 2009-2010 school calendar.  Please note: Student orientations will begin the WEEK OF AUGUST 31st – WITH A NEW PARENT SOCIAL ON AUGUST 30^TH FROM 5:30 – 8:00 P.M. AND FRESHMAN ORIENTATION ON SEPTEMBER 1st  FROM 8:00 a.m. to 1:00 p.m.)  For immediate information on the upcoming school year, please visit our website at www.cheverus.org
