As
we head into the summer season, I hope that you are beginning to relax and
enjoy your family away from the sometimes overwhelming structure of the
academic year. Before you do settle into summer, I would ask that you please
read this letter and keep it for reference, as it does contain information that
is critical to the start of the new school year.
Cheverus has mailed you an Emergency
Information Form. Please carefully check the printed information and make
any necessary changes. (Be sure to include a telephone number where you can be
reached in the event of an emergency and two other non-parent contacts.) If you
would like to be contacted by e-mail, please provide us with your email address
as well. (Since Emergency Information
is vitally important, as is proof of physical examination, we must ask that you
provide the school this information no later than July 16th) No student will participate in any sport or
activity or view their course schedule on Netclassroom until all required forms
have been returned to the school. Thank you very much for your help.
Instead
of mailing you a cumbersome packet of additional information, we have made the information
accessible to you on the Cheverus web site. Click here to download the Cheverus Forms. For other
important information, review the links at the bottom of this page. Although all of the information made
available to you on our website is important, I have listed below other documents
that require your immediate attention:
SCHOOL POLICY requires that
all freshmen, juniors, and transfer
students provide documentation of a physical exam. Cheverus encourages
using the athletic Pre-Participation Physical Form available on our
website. The documentation will be filed in the student’s Permanent School
Health Record.
Athletic Pre-Participation Physical
Form The Athletic Department requires that
any student who plans to play a sport during the 10-11 school year, have
on file a current completed physical form(physicals are good for two
years). This form can be found under “Forms” or under “Athletics” on our
web site. The fall sports pre-season begins on Aug. 16th.
Substance Abuse Contract (Must be
signed by all freshmen and transfer students) NO STUDENTS WILL PARTICPATE IN ANY ACTIVITIES OR VIEW THEIR FINAL COURSE
SCHEDULE BEFORE SIGNING AND RETURNIG THIS DOCUMENT.(Due July 16th)
Parent Directory Form Sign and return this document if you do not wish to be included
in the Student Directory.
Please

be well prepared to start the new school year.
NOTE: The 2010-2011 school calendar is now
available. Student orientations will
begin the WEEK OF AUGUST 30th. FRESHMAN ORIENTATION IS ON AUGUST 31st from 8:00 a.m. to 12:30 p.m.) THE
NEW PARENT SOCIAL WILL BE ON SEPTEMBER 9th FROM 5:30 – 6:30 P.M.
